The first thing to do is off course seek medical attention if there is a life-threatening injury.
If things are not so dire I recommend taking a picture of the condition (debris, fallen ladder, etc.) that caused the accident.

Be very specific as to what you tell the doctors, investigators and anyone else. If a ladder slipped when you were working on it say exactly that! Don’t just say “I fell off a ladder”.
